Open Position: Habilitation Specialist – RESIDENCE. FT 40 hours per week

Location: Sands Point, NY.
Evening Shift. A Habilitation Specialist for the DBIE provides functional and person-centered support to the participants in the week long DBIE. Responsibilities include offering exposure and training activities related to the domains of home, community and work, documentation, participating in meetings and interfacing with all members of the home team. This person works a varied schedule and may support the residents at Destiny home in all aspects of their personal and home management, community interactions as well as at paid work sites. Ability to travel is required to provide week long consultations with the DBIE team. Specific duties include: supports students in the following areas: personal grooming, communication, socialization, home management, vocation, community skills and behavioral support under the guidance of the DBIE coordinator. Provides consistent regular checks on all students during shift, ensuring safety and well being. Accompanies students to the hospital when needed; Participates in team meetings and assists residents with safe evacuation in the event of an emergency. Participates in regularly scheduled drills to prepare for such emergencies; Participates in regular in-service training; Maintains daily, weekly and or monthly observation reports as required. Accompanies and serves as a guide and/or interpreter for students and professional staff members, when assigned. To accomplish this, it may be required to use personal vehicle on occasion. Dispense consumer medications after passing the AMAP course. Must become AMAP certified and maintain this certification as a condition of continued employment in this position. Required to obtain CPR and First Aid certification. Ensures compliance with OPWDD regulations. Qualifications: Associates Degree, or completion of two years of college (60 credits) preferably within the education or health related field of study required OR High school diploma and four years of paid experience as a residence/group home aide working with individuals who have intellectual disabilities required. One year of experience in work with individuals who are deaf-blind, blind, deaf or have intellectual disabilities. Must possess a clean, valid New York State Driver’s license. Knowledge of MS Office and Internet required. Category D.
